*Develops a written plan of care for each patient, assessing the physical, psycho social needs of the patient and documents findings.
*Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary
*Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
*Performs patient care responsibilities considering needs specific to the standard of care for patient’s age.
*Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical position.
*Provides skilled nursing care.
*Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
*Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
*Initiates case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
*Follows up with obtaining lab results and notifying physician of any significant results.
*Re-evaluates the need for continued care on an ongoing basis; initiates recertification as necessary.
*Plans for discharge from the Agency with the patient and caregivers. Makes referrals for alternative level of care if necessary.
*Promotes patient/family/caregiver education using various verbal and written communication techniques that take into account the patient’s/family’s cultural, ethnic and/or personal needs or preferences.
*Supervises Home Health Aides on an every other week basis.
*Initiates needed referrals to other disciplines and/or community resources.
*Obtains needed supplies and equipment to provide care.
*Knowledgeable of current federal, state and agency regulations concerning patient care.
*Adheres to all Agency, payroll, billing and documentation policies and procedures.
*Must participate in on-call rotation as dictated
*Must perform RN visits as scheduled in all service areas
*Completes and submits all notes/documentation within 48 hours of visit.
Adheres to dress code, appearance is neat and clean.
Completes annual education requirements.
Maintains regulatory requirements.
Maintains patient confidentiality at all times.
Reports to work on time and as scheduled, completes work within designated time.
Attends annual review and department inservices, as scheduled.
Represents the organization in a positive and professional manner.
Actively participates in performance improvement and continuous quality improvement (CQI) activities.
Complies with all organizational policies regarding ethical business practices.
Communicates the mission, ethics and goals of the organization.
General Background and Minimum Requirements:
Diploma, Associate or Baccalaureate degree in nursing
Current unrestricted RN license in Texas
Current CPR certification
One year RN experience
Home Health experience is preferred but not required
Must have valid state driver's license, reliable transportation and auto insurance
Must have the ability to function autonomously and possess effective time management skills
