The goal of patient navigation in the Emergency Department is to improve health outcomes and patient experience at a reduced cost. The role of the ED Patient Navigator will be to facilitate patients with multiple medical and social needs navigate through the healthcare system to receive high quality and timely care in the most appropriate setting.

Responsibilities:

MAJOR TASKS DUTIES AND RESPONSIBILITIES:

  • The Patient Navigator will work with the multidisciplinary hospital team to conduct an assessment of the patient’s health and social needs and identify current gaps or barriers in accessing healthcare.
  • The Patient Navigator will review patient care plans to ensure evidence based best practices are implemented.
  • The Patient Navigator will ensure that patients enrolled in the navigation program have completed medication reconciliation in place.
  • The Patient Navigator will organize post-discharge services. The Navigator will make follow-up appointments with physicians, organize adjunct therapies (home health, physical therapy, cardiac rehab, DME, etc.), and arrange any pending tests or studies to be reported to the follow- up physician. The patient Navigator will ensure that each patient enrolled in the navigation program has an appointment scheduled with a primary care physician. The Navigator will work with the patient, family and hospital team to arrange post-discharge services.
  • The Patient Navigator will ensure that health information is communicated to primary care providers.
  • The Patient Navigator will educate patients about their disease process utilizing the teach-back method to confirm the patients/families understanding. The Patient Navigator will also provide the patient/family with self-management tools and resources.
  • The Patient Navigator will provide patients with referrals to outpatient and community resources.
  • The Patient Navigator will provide telephone follow-up to high medical and social need patients 2-3 days after discharge.
  • The Patient Navigator will assist in measuring data to support the Emergency Department Patient Navigation program goals. The Navigator will participate with performance improvement activities to improve health outcomes and patient experience at reduced costs.

  • Qualifications:

    EDUCATION

  • Required: Graduate from an accredited school or college of nursing.
  • Required: Current Texas Registered Nurse licensure.
  • Required: Current CPR certification


  • EXPERIENCE:

  • Required: two years in acute care nursing position.
  • Preferred: Emergency Department experience.
  • Demonstrates knowledge of quality and performance improvement principals
  • Effective written and verbal communication and negotiating skills with all levels of internal and external customers.
  • Must demonstrate positive relationship building with patients, peers, medical staff and other members of the clinical care team.
  • Must demonstrate the ability to effectively implement changes
