POSITION SUMMARY:

Responsible for providing patient/family care in consultation with primary physician, team manager, and interdisciplinary team of internal and external partners in accordance with Optio Health Services policies and philosophy of care. Provides appropriate nursing interventions and documentation. Assist patient/families with external resources such as home health, financial concerns, counseling, etc. Educates, empowers, and motivates patient and families regarding actions and behaviors towards self-care, wellbeing and self-empowerment.

ESSENTIAL DUTIES: Ø

Provides nursing coordination of care to patient/families as determined by Plan of Care. Use education, mentoring, guidance and interventions to achieve goals of care with strong patient and family focus. Develops care plan with patients/families, primary physician and IDT. Strong commitment to partnering with patient and their families to develop care plan that is consistent with their culture, abilities, and goals. Ø

Provides accurate and timely documentation in Electronic Medical Record. Uses clear, concise, and professional written and verbal communication. Ø

Provide basic assessment and demonstrates respect for persons with severe/persistent mental illness with chronic diseases. Uses sound nursing judgment and strong assessment skills to perform complex telephone interviews and triaging as appropriate. Assist patient/family with Advanced Care Planning. Ø

Comfortable with visits in a variety of settings including home, skilled nursing, hospital, or other settings with combination of in-person and telephonic interventions. Ø

Participates in Optio Health Services and related services, such as community education, team training, staff/board committees and orientation of employees. Participates in staff meetings, in-services, and other sponsored functions. Ø

Work independently with minimum direction, anticipate and organize workflow, prioritize and follow through on responsibilities. Develop and organize work plan in a high volume environment with rapidly changing priorities. Ø

Participates in continuing education and professional development. Ø

Reliable transportation with ability to travel to patient/families within our services area. Regular and predictable attendance. Provides on-call services as required. Ø

Other duties as assigned.

EXPERIENCE:

MINIMUM: Ø

2 years experience in behavioral health nursing, hospice/palliative care, med/surg, public health, oncology, case management, chronic disease management, primary care, acute care, or home health.

PREFERRED: Ø

Baccalaureate degree in nursing Ø

4+ experience as a Nurse Ø

Case Management nursing Ø

1 year experience working in community based setting Ø

Chronic disease management experience Ø

Behavioral health nursing
