PURPOSE OF POSITION: This person will, within the YVFWC’s department of Northwest Community Action Center (NCAC), provide professional level case management services to the targeted clients of the Mid-Valley Providers Consortium under the Readiness to Learn grant.
Readiness to Learn Case Manager will work on site in the community of Sunnyside and Outlook in partnership with school district staff to assist students in overcoming non-academic barriers to academic success. Case Management includes providing social & health service referrals that will assist in removing barriers to student learning; providing critical links between schools, home and providers; advocating for student and family needs; maintaining accurate and appropriate documentation and adhering to confidentiality protocols; participating in networking and development of needed community resources.
This position includes community mobilization work in the community of Outlook. This includes organizing community meetings and events that will occur in evenings and on weekends.
Education: Bachelor’s Degree in related field or comparable experience in working with youth parents, school personnel, and/or provider agencies.
Licenses/Certificates/Registration: Current Washington State driver’s license and proof of automobile liability insurance coverage.
Knowledge/Skills/Abilities Required: Bilingual (English and Spanish) preferred; knowledge of human relations, and communication skills with ethnic minorities; knowledge of human service programs and the ability to develop effective working relationships with residents, human service providers, co-workers and the public; ability to work evenings/weekends and attend meetings as required.
