JOB TITLE: REGISTERED NURSE TEAM LEADER - WEST PINAL SITES |
REPORTS TO: DIRECTOR OF NURSING/DIRECTOR OF OPERATIONS
STATEMENT OF PURPOSE:
Under general supervision, plans, directs, and evaluates total nursing care and functions, in accordance with established philosophy and reflective of Sun Lifes Mission and Vision. The RN Team Leader reports to the Director of Nursing for all clinical issues and the Director of Operations for all other matters. Assists with orientation, training, and management of personnel. Relates effectively and maintains satisfactory relations with other departments; participates in all phases of education, maintenance of records, and skills of personnel. Will coordinate care for patients on the providers daily schedule and proactively manage and coordinate care for patients not on the schedule.

Identify patient service requirements by establishing rapport with the patient and others while being aware of age specific needs protocols. Exhibit knowledge of growth, development and age appropriate communication skills. Ability to assess, provide and interpret age specific data. Ability to involve family or significant other in decision-making related to plan of care.
Promote teamwork within the site to improve patient access and the efficiency of the team and the site as a whole.
Review current or new plan of care to include goals with patient, Perform patient education activities as directed by provider.
Provide the best possible nursing care by planning, organizing, and directing the nursing functions of patients.
Identify problems and guides personnel to their solution.
Identifies patient service requirements by establishing rapport with the patient and others while being aware of age specific needs protocols.
Set up rooms, place patient in room, take patients vital signs, height, weight, pulse ox, oxygen levels, etc. and record them in the chart, prepare patient for examination by the provider, assist with performing EKGs on patient, assist the provider with patients care, and apply braces, ace wraps and wound dressings.
Triage patients and record appropriate documentation in patient chart, in a timely and professional manner.
Complete patient care requirements by performing tests and completing other tasks as assigned by the Director of Nursing and/or the provider. Assist provider in examination of patients, and give instructions to patients as directed by provider and accurately and promptly implement providers' orders.
Monitor medical condition of patients and report changes to provider.
Administer and document medications, immunizations, and monitor intravenous solutions skillfully and correctly. Fill out appropriate immunization paperwork for recording purposes and document in ASIIS.
Answer telephone calls and take messages for the provider and/or return telephone calls. Act as a liaison with the providers, other doctors offices, and the hospital including receiving reports on patients and delivering them to the appropriate provider.
Clean and prepare rooms between procedures, and restock supplies noting any needing reordered in inventory. Perform cleaning, sterilization of instruments, and autoclaving packs in a timely manner.
Process referrals (outlying sites), medication refills, and set up appointments for patient for diagnostic procedures and/or with a specialist. Arrange for patient to receive home health visits, if necessary. Schedule patient for diagnostic testing, i.e. radiological testing, appointment with a specialist and instruct staff in computer documentation and follow-up.
Notify patient of test results when instructed to do so by provider by phone or letter. Act as a liaison with the providers, other doctors offices and the hospital and arrange emergency operations and other services.
Assure quality of care by maintaining clinic standards of care while adhering to requirements of other governing agencies including reporting communicable disease to County Health Department as needed.
Work with staff to become and maintain recognition with the Nation Center for Quality Assurance as a Patient Centered Medical Home.
Protect patients and employees by complying with infection control policies and protocols, following medication administration, storage procedures and controlled substance regulations and monitoring staff for compliance on these issues.
Participate in patient education, i.e. family planning, diabetes, well child, medication etc.
Call in prescription refills as directed by the provider.
Maintain documentation of patient care services by completing patient and department records in accordance with established policies and procedures.
Ensure operation of medical equipment by completing preventive maintenance requirements; following manufacturers instructions; reporting malfunctions, requesting repairs, maintaining equipment inventories, and evaluating new equipment and techniques.
Maintain nursing supplies inventory by anticipating needed supplies. Keep rooms stocked with adequate medical supplies, wash and dry instruments used, and prepare for sterilization as required.
Perform venipuncture, specimen collection and laboratory CLIA waived testing for which he/she has received training.
Participate in orientation and in-service training for personnel.
Evaluate the performance of personnel with recommendations for appropriate action. Keep Director of Nursing/Director of Operations informed of staff needs, and problems.
Maintain Clinic standards of care by adhering to the Arizona State Nurse Practice Act and those of other governing agencies. Adhere to expectations of required/mandatory in-services, certifications, and licensure.
Directly supervise Registered Nurses, Licensed Practical Nurses, Medical Assistants, Referral Clerks, Front Office personnel and students on site and report to Director of Nursing/Director of Operations any issues in a timely manner.
Provide staff education, orientation, training, monitoring and in-services and make assignments to the skill level of employees.
Interact with patients and families in a tactful manner and refer patient grievances/complaints to Director of Nursing/Director of Operations to be addressed.
Foster a working environment that promotes growth, a team environment and job satisfaction of personnel.
Assist Director of Nursing/Director of Operations with the maintenance of training files, competencies and maintenance of employee health records and screening of employees.
Identify problems and make recommendations to Director of Nursing/Director of Operations for solutions in day to day clinic operations.
Conduct monthly Safety audits and participate in Wellness monitoring activities.
Act as a liaison between providers, site staff and Director of Nursing/Director of Operations to communicate any issues needing attention.
Cooperate and maintain good rapport with providers, staff, other departments and visitors.
Assist personnel with computerized timekeeping system and ensure all staff have hours correct and approved in a timely manner.
Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; and establishing personal networks.
Participate in in-service/education regarding Quality Improvement.
Participate in planning changes and improvements.
Perform all other related duties as assigned or requested.

EDUCATION AND EXPERIENCE:
• Registered nurse (R.N.) with valid state license
• ADN, AAS degree required
• One year of nursing experience as an RN or LPN required
