Summary of Functions:
Evaluates the effectiveness of
care in conjunction with other members of the interdisciplinary team.
Reports changes in patient condition to the attending physician and seeks
needed changes in pharmacological intervention as warranted. Revises the
plan of care to respond to evaluation of effectiveness.
Performs the assessment function
for all adult clients and derives nursing diagnoses based on assessment

collaboration with the interdisciplinary team to resolve current problems
in support of the dying patient, family and caregivers. Involves the
patient/family in determining care goals and objectives. Initiates change
in level of care as appropriate to meet the needs of the patient and

to relieve symptoms associated with pain, disease and the normal dying
process. Delivers care within the scope of nursing practice and in
accordance with accepted standards of practice.
Demonstrates and instructs care to
be provided by the patient, family, or caregiver and provides ongoing
supervision and instructing as warranted. Provides emotional support to
patients and families in a manner that fosters trust and comfort to
resolution of concern/needs. Respects the autonomy and right to
confidentiality of all patients and families.
Assesses the need for spiritual
care, volunteer and home health aide services. Plans, teaches and supervises the
provision of home health aide services. Provides on-site orientation and
supervision of home health aides on an ongoing basis. Documents and reports to
appropriate supervisor any identified performance issues.
Identifies need for intervention
by other members of the interdisciplinary team on an ongoing basis;
collaborates in the planning of such care. Reports changes in patient
condition to the managing physician and seeks changes in pharmacological
intervention or plan of care, as warranted. Documents effectiveness in
response to the plan of care. Recognizes elements of patient and family
systems that are not amenable to change. Identifies the need for
recertification of hospice services, provides complete and accurate
supporting documentation, and seeks physician order to recertify the

care settings through effective information management.
Participates in the orientation of
new members of the interdisciplinary team. As requested, makes joint
observation visits with residents, employees, community agency
representatives and students.
Identifies and articulates
essential components of hospice health benefits as administered under
Medicare and other payers and demonstrates an understanding of hospice
health reimbursement methodologies to manage a caseload and deliver care
in a cost-effective manner.
Participates in the development of
policies, procedures, clinical protocols, forms, and service programs as
requested. Participates in departmental and/or agency-wide quality
improvement activities.

Minimum Education and Experience:
degree in Nursing.
1 year of
current home health care or hospice experience.

Minimum Requirements:
unrestricted license to practice as a registered nurse.
driver's license, reliable transportation, and current, valid
automobile insurance.
