Northeast Parent & Child Society’s Community and Home Integration Project (CHIP) invite applications for a Service Navigator.

The Service Navigator provides assessment; direct clinical services, clinical case management and service coordination with community based prevention services and needed community resources for the youth and families in the CHIP program. This includes: developing, implementing, and monitoring therapeutic components of the daily program at CHIP; providing direct clinical services; coordinating community services for the family; and interfacing with the youth’s school program. Clinical services include: assessment; treatment planning; individual, group, and family counseling; case management, and facilitating the treatment team.

Skills/Qualifications: Master’s degree in Social Work, Counseling, or related field. License preferred. Experience working with families and youth in a community setting. Experience with home based services and youth on probation preferred with the ability to work independently.

Work Schedule: Monday-Friday, some evening work to meet families’ needs.
