Perform patient initial evaluation completely and competently, in accordance with established practice standards and facility policies and procedures.

Accurately summarize objective clinical findings.

Develop an accurate individualized plan of care.

Develop feasible, measurable goals based upon findings and patient/caregiver expectations.

Complete recertification of plan of care timely and completely.

Complete all orders including initial evaluation, recertification, changes to plan of care, and discharge, timely and in accordance with facility policies, procedures and in collaboration with physician.

Provide effective direct patient treatment in accordance with established plan of care and physician orders.

Implement appropriate preventative measures, home exercise or restorative nursing programs, adaptive equipment, etc. to sustain or prevent decline in patient's functional status.

Assess educational needs of the patient, family, or caregiver and provide appropriate education to meet those needs.

Effectively communicate/collaborate with patient, family, caregivers and other members of the healthcare team to promote maximum benefit of care.

Complete all clinical documentation completely, accurately, legibly and timely in accordance with regulatory requirements.

Make appropriate discharge plans in collaboration with IDT.

Effectively completes patient screens.

Demonstrate appropriate knowledge of clinical techniques, procedures and programs, use of available modalities and equipment for clinical setting and patient population served.

Effectively use discretionary time to perform tasks beneficial to the rehabilitation department.

Meet established productivity standards.

Appropriately manage physical resources of the dept by demonstrating fiscal awareness and respectful and proper use of equipment.

Demonstrate consistent knowledge and application of Medicare and other reimbursement regulations.

Seek professional growth and development through continuing education, mentoring opportunities, learning and applying new skills, etc.

Required Skills

Current state Speech & Language Pathologist licensure.

Current ASHA certification.

Must be able to speak, read, write and understand English.

Must be organized, self-directed and highly motivated.

Patience, tact, enthusiasm, and positive attitude toward the elderly.
