Primary Function & Major Responsibilities The SW Care Manager is responsible for Psychosocial care management ensuring the achievement of desirable patient outcomes, appropriate length of stay, efficient utilization of resources, increased patient and family involvement, and patient/staff/family education. Implementation is accomplished through patient/family psychosocial assessment, monitoring of the plan of care, review activities, coordination with the interdisciplinary team and any outside third-party payers, communicating with physicians, reduce avoidable delays and control costs where possible, and by facilitating post-hospital discharge needs including nursing home placement, financial assistance, counseling and other community resources.Develops, coordinates and implements psychosocial care management through partnership with RN Care Managers and Care Management Assistants in collaboration with Clinical Leadership, Physicians, Nursing staff, and other Interdisciplinary Clinicians.Provides psychosocial support.Facilitates plans for the transitions of patient care to the next level and location of care. Partners with system Care Navigation resources and care navigators from other settings provide continuity of care and effective transitions.Works in partnership with the RN Case Manager to ensure timely patient discharge.Participates in professional development activities.Demonstrates flexibility and teamwork behaviors among social work staff members.Performs basic administrative tasks including data management and other related duties as required.Maintains confidentiality in all aspects of the job.Performs all other related duties as assigned.
Bachelor’s degree in social work from an accredited school of social work.
One year hospital social work experience or two years nursing home experience.Special knowledge,
Skills and Abilities:
Knowledge of applicable laws and regulations;Knowledge of community resources;Knowledge of medical setting and basic medical terminology;Ability to utilize strong oral and written communication skills;Demonstrates the ability to interact skillfully with people;Demonstrates ability to exercise independent judgment;Demonstrates ability to identify need for quality improvement and suggest solutions;Must able to demonstrate the knowledge and skills necessary to provide care appropriate to all age groups (adolescent through geriatric). Must demonstrate knowledge of the principles of growth and development through the life span, posses the ability to assess data reflective of the patient’s status, and interpret the appropriate information needed to identify each patient’s needs in relation to age and to provide the care needed as described in the department’s policies and procedures.Regular/Temporary
