Position will provide support for Secure Health plan members’ annual comprehensive Medical Conferences within a defined service area.
The Social Worker helps identify appropriate providers, facilities and services for the Secure Health member from the time of clinic exam and any transition throughout the continuum of care while ensuring that available resources are being used in a timely and cost-effective manner in order to obtain optimum value for the member, providers, and plan.
Coordinate various clinic dates and social work presence with all locations
Manage data provided by member during Secure Health appointments
Distribute education materials based on member’s chronic conditions and community resources.
Serve as an advocate and educator for Secure Health members for questions related to healthcare needs and health plan navigation.
Conduct a thorough and objective face-to-face evaluation of the member’s current psychosocial status including physical, environmental, financial, and health status expectations.
Establish a plan of care or update the established plan of care as needed.
Address Secure Health members’ minor social needs/needs that can be managed within a clinic visit
Identify opportunities for intervention that include referrals to community resources.
Screen Secure Health members for referral to other care management services such as Outpatient Social Worker, Case Management, and Chronic Care Improvement Programs.
Set goals and time frames for goals appropriate to the individual.
Effectively communicate goals and time frames to members and their families/support systems, physicians, On-site Review nurses, and (when appropriate) PHN Case Management.
Arrange and monitor appropriate services for members. Follow up with members as needed in relation to their plan of care.
Provide support of other initiatives when there is time. Including but not limited to SNP telephone assessments and Health Fairs.
Perform other job related duties as requested
Master of Social Work from an accredited graduate program.
Knowledgeable and experienced in computer skills.
Excellent interpersonal, verbal and written communication skills.
Ability to work independently and proactively, and make decisions.
Ability to develop and maintain a working relationship with Peoples Health Network staff.
Ability to coordinate a complex schedule to ensure clinic coverage

Master of Social Wok degree from an accredited graduate program required. CSW (with graduate work experience in a health-care setting) or LMSW required. LCSW or LCSW eligible (currently under supervision) with a minimum of professional work exposure or graduate field practicum in either medical social work or mental health tracks highly preferred. Excellent interpersonal, verbal and written communication skills are required. Must be able to work independently and pro-actively, and make decisions. Ability to handle multiple demands of diverse workload and prioritizes critical issues. Excellent time management skills, the ability to analyze and think critically, needed.

Physical Demands/Working Conditions
Occasionally lift and carry items weighing up to 40 pounds. Requires a full range of body motion enabling the incumbent to handle and assist in lifting patients. The work environment involves moderate risks or discomforts, which require special safety precautions, e.g., working with risk of exposure to contagious disease or infection, or working with emotionally, disturbed patients. Precautions are routine for nearly all situations. Be able to travel to various locations as needed.
