This professional position provides case management services and service coordination for families involved in the Frederick County Infants and Toddlers Program (FCITP). FCITP provides early intervention services to families with children (birth to 5 years) who are experiencing developmental delays. Supervision is received from the Infants and Toddlers Program Supervisor and/or FCDC Director.

Examples of Work:
  • Coordinate early intervention assessments and services for caseload of children and their families
  • Complete intake process, including participation in assessment and develop initial Individual Family Service Plans with service providers and families.
  • Complete six-month and annual reviews of Individual Family Service Plans for child/family caseloads consistent with mandated timelines
  • Assist families in assessing their needs for support/resources with respect to promoting their child’s development
  • Educate families about resources within the agency, community and state, as needed and coordinate linkages
  • Participate in team meetings for purposes of coordinating services and planning treatment
  • Provide support to team members, both formally and informally, as they strive to develop and maintain effective working relationships with family members or caretakers or referral sources
  • Provide family members with education and guidance regarding social-emotional development, developmental delays, parenting and family functioning as may be impacted by challenges of a child experiencing developmental delays or complex medical needs
  • Co-lead parent support groups and assist in organizing periodic family-centered events
  • Participate in departmental and interagency meetings to promote optimal agency functioning
  • Promote integration of the program’s family-centered philosophy and developmental model of intervention
  • Provide service coordination activities in natural environments and by telephone
  • Coordinate transition of children from Infants and Toddlers Program to Frederick County Public Schools through Child Find and other community resources
  • Perform other related duties as required

Qualifications & Requirements:
  • Bachelor’s degree in Social Work, Psychology, Counseling, Nursing, Family Services, Education or a related field
  • Minimum 3 years of relevant pediatric, case management and/or service coordination work experience – a related Master’s degree may reduce the experience requirement to 1 year
  • Working knowledge of developmental disabilities and knowledge of infant and early childhood development
  • Knowledge of the development and implementation of Individual Family Service Plans
  • Ability to develop and maintain effective working relationships with co-workers, children who have developmental delays and/or disabilities and their families, and the general public
  • Ability to effectively function as a member of an interagency team
  • Ability to effectively organize work, establish priorities and complete assigned duties with minimal supervision
  • Strong and effective spoken and written (English) communication skills
PREFERENCE MAY BE GIVEN FOR:
  • Experience working with families of infants and toddlers with developmental disabilities
  • Medical case management experience
  • Knowledge of Part C of the Individuals with Disabilities Education Act (IDEA)
  • Bilingual (Spanish/English or American Sign Language/English) proficiency
  • Current MD State licensure as a Social Worker, Psychologist, Psychology Associate, Professional Counselor, RN or LPN
