Volunteers of America Texas is a faith based, non-profit organization dedicated to helping individuals, families and communities reach their full spiritual, emotional and physical potential and wellbeing through a vast array of services that target the elderly, the intellectually disabled, those striving to overcome substance abuse, and those transitioning from correctional centers.

Function:

The Service Coordinator is responsible for providing general social service coordination (including intake) and referral services to all residents needing such assistance.

Primary Responsibilities:
  • May provide informal, individual assessments (i.e., evaluation of health, psychological and social needs, development of an individually tailored case plan for services and periodic reassessment of the resident's situation and needs) for a resident when such service is not available through the general community.
  • Establishes linkages with all agencies and service providers in the community; shops around to determine/develop the best "deals" in service pricing, to assure individualized, flexible, and creative services for the involved resident(s).
  • Sets up a directory of providers for use by both project staff and residents.
  • Refers and links the residents of the project to service providers in the general community. These are, for example, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness and legal advocacy.
  • Educates residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate.
  • Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual. Manages the provision of supportive services where appropriate.
  • Helps the residents build informal support networks with other residents, family and friends.
  • May set up volunteer support programs with service organizations in the community.
  • May provide training to project residents in the obligations of tenancy or coordinate such training.
  • May educate other staff on the management team on issues related to aging in place and service coordination, to help them to better work with and assists the residents.
  • Financial Management: Within the scope of responsibility, adheres to budget guidelines, including expenditures, billing for services, P-Card documentation, etc.
Qualification Requirements:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily and must successfully complete any required training and acquire any required certifications within prescribed timeframes. The position requires demonstrated clerical skills, including typing at 30 WPM. It requires a reasonable combination of the following skills and abilities: oral and written communication, work independently, attention to detail, dealing with public, organization and operation of office machines and computers. Ability to access sites and attend HUD training sessions.
  • Skill in the use of a personal computer and software applications (e.g. Microsoft, Word, Excel, Access, PowerPoint, etc.).
  • Strong organizational skills and the ability to maintain detailed records.
  • Ability to communicate effectively both written and verbally.
  • Ability to speak effectively before groups of consumers or employees of organization.
  • Ability to work effectively under stressful conditions.
  • Ability to work independently with little supervision.
  • Ability to exercise initiative and sound judgment and to react with discretion under varying conditions.
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Ability to recognize problems and take corrective actions as determined by agency and program policies and procedures.
  • Ability to define problems, collect data, establish facts and draw valid conclusions.
  • Ability to interact with staff members and community contacts

Other Requirements:
  • Bachelor's degree in Social Work or related field; two years in social service delivery with senior citizens and non-elderly disabled preferred.d.
  • Valid Texas Drivers License (Must be received within 30 days of employment) and ability to be insured; valid Vehicle Liability if driving personal vehicle.
Salary:
Hourly pay rate beginning at $13.00 to $15.00 per hour.
