A. MAJOR FUNCTION:
Under the supervision of the Office Director provide assessment services to clients and their families.
B. MAJOR SPECIFIC RESPONSIBILITIES:
Conduct the assessment process for new clients.
Maintain client charting in accordance with ODADAS, ODMH and professional standards.
Participate in all assigned staff meeting and staff development programs.
Maintain clear communication with referents or designated liaison person.
Maintain proper clients rights and confidentially.
Participate in all Quality Assurance and Utilization Review activities.
Complete other miscellaneous duties assigned from time to time by supervisors
C. RESPONSIBILITY AND DECISION MAKING AUTHORITY:
Responsible to make decisions concerning client treatment such as diagnosis, level of care referrals and recommendations. All other activities are to be conducted with the consultation and assistance of the Director of Clinical Operations. At all times treatment activities should be performed as a team function.
D. MINIMUM REQUIREMENTS:
One of the following credentials: RN, LSW, LISW, PC, PCC, Licensed by the Ohio Counselor and Social Worker Board or for chemical dependency assessments only-Certified Chemical Dependency Counselor (Level III preferred)
Posses a working knowledge of; the disease concept of chemical dependency, chemical dependency treatment, Alcoholic's Anonymous philosophy, and group dynamics.
