PRIMARY PURPOSE AND JOB FUNCTION
Working in accordance with established policies and procedures and/or specific instructions from the Social Services/Admissions Director coordinates or performs the necessary administrative functions regarding the admission/discharging of residents in the Health Center and assisting in planning, organizing and coordinating the Social Services provided for residents of the facility. This position is also responsible for providing information, direction and other assistance to residents, guests and staff and conducts tours of the Health Center and assists with inquiries for Health Center admissions. Presents a positive, courteous, and professional image of self, as well as facility.
This position is under the direction of the Social Services Director
DUTIES AND RESPONSIBILITIES
Essential Job Functions:
During emergencies or natural disasters, employee may be required to report for duty as assigned by the Administrator or her designee, for the duration of the disaster.
Maintains acceptable standards of personal hygiene and complies with department dress code.
Attends in-service training and education sessions as assigned.
Identifies the medically related social service and emotional needs of the resident.
Provides Social Services to residents requiring such services or makes referrals to Social agencies if necessary.
Documents residents social history, MDS, Raps and assessments..
Helps with connecting telephone in Resident rooms.
Arranges newspaper service for Residents if needed.
Assists in and completes admission paperwork including gathering of information.
Assists in meeting Residents psychosocial needs. Evaluates for concerns.
Performs shopping tasks for materials.
Attends Care Plans for resident families or Responsible parties, also any commities assigned to.
Non-Essential Job Functions
Report any suspicious activities or persons.
Performs specific work duties and responsibilities as assigned by supervisor.
FACTORS AND SUBSTANTIATING DATA
Education: Bachelors degree or higher in appropriate discipline required. Must have the ability to read, write, have good computer skills and speak English. Highly computer literate
Experience: Must possess the ability to interact with the public in a professional manner as well as be able to work with a variety of people and function as a member of the health care team. Prior experience in Social Service related positions in Long-term Care setting.
Complexity: A wide variety of duties and responsibilities involving the ability to provide detailed information, have auditable handwriting, handle a multiplicity of tasks including, but not limited to, the ability to discuss financial situation with responsible party and make determinations regarding admissions based on financial statements.
Supervision Received: Under the direct supervision of the Social Services Director
Contact Others: Frequent contact with residents, guests and staff. Requires a cooperative and courteous attitude despite simultaneous tasks. Must have the ability to be kind and understanding when dealing with the resident. Must have the tolerance to perform a variety of duties under frequently stressful circumstances. Contact Social Services of discharge planners in hospitals and other health care facilities in admissions of transfer and discharge activities.
Errors: Errors could result in service delays resulting in resident dissatisfaction, poor communication and possible loss of revenue to the community.
Confidentiality: Will be exposed to confidential information whose disclosure would be contrary to the best interest of the residents, the Palms of Sebring and professional ethics. Actively promotes resident confidentiality and rights.
Sensory Demands: Ability to maintain a cooperative and courteous attitude despite simultaneous demands. Must have the tolerance to perform a variety of duties under frequently stressful circumstances. Must be able to function efficiently despite distractions and interruptions and be able to remain calm in an emergency. Deadlines and a number of simultaneous demands can produce stressful situations on occasion.
Physical Demands: Must have the ability to perform gross and fine motor skills. Must have good hearing and visual acuity. Must meet time line compliance.
Working Conditions: Works in a well lighted office. Exposed to normal office noise. May be exposed to offensive odors and unattractive sights. Works in a nursing facility environment with regular exposure to chronically ill and frail elderly.
Scope of Supervision: No supervisory responsibility.
Travel: Position requires some outside local travel.
Safety: Requires ability to understand and follow policy and procedures of infection control, disaster preparedness, safety rules and regulations and department rules.
Hazards: This position is occupationally classified as being at risk for exposure to blood borne pathogens.
Equipment: Will be exposed to various types of equipment found in a health care environment.
