About the Opportunity DIRECTOR OF SOCIAL SERVICES Magnolia Place of Greenville, a 120 bed skilled nursing home is seeking a talented and dedicated Director of Social Services to lead our Social Services team! About the Opportunity Below is the “normal" stuff that everyone posts but to get to the heart of the matter (which is what the Social Worker does…) is this. The Social Worker is the person who works directly with our residents and their families. This job requires the Social Worker to have an enormous amount of patience and compassion because working with families admitting their loved ones into long term care is a stressful life event – its everyone’s job to make that transition as smoothly as possible but as the “heart" of the facility – you will play a major role in this. The Social Worker is responsible for assisting in the planning, developing, organizing, implementing, evaluating, and directing the Social Services Department at our facility in accordance with current existing Federal, State and local standards as well as our established policies and procedures to ensure that the medically related emotional and social needs of patients and/or residents are met and maintained on an individual basis. Develops and manages initial contact as necessary with discharge planner, family or responsible party to initiate tour of facility and follow up procedures. Function: Meet with administration, medical and nursing staff, and other related departments in planning social services. Develop and maintain a good working rapport with intra-department personnel, other departments within the facility and outside community health, welfare and social agencies to ensure that social service programs can be properly maintained to meet the needs of the patients/residents. Keep abreast of current federal and state regulations as well as professional standards.

Qualifications Qualifications • Ideal candidate must be a skilled communicator, director and motivator. Able to organize and prioritize many tasks effectively. • Minimum of two years experience in a long term care, hospital or other related medical facility. Long term care experience preferred • Able to react to emergency situations appropriately when required. • Bachelors degree – Social Work • Must be a Licensed Social Worker - Texas
