The purpose of this position is to provide services to meet the social and/or emotional needs that affect the residents' ability to achieve their highest level of function.
Develop policies and procedures to provide social services to residents in compliance with federal, state and local regulations.
Interview, assess and develop individualized plans to meet residents' needs.
Assess the facility's social service program and make recommendations to the administrator.
Develop and implement social services policies and procedures to meet residents' needs in compliance with federal, state and local regulations.
Develop, coordinate and participate in family and resident activities designed to promote social interaction, reality orientation and intellectual stimulation.
Develop one-to-one professional relationships with residents and families as needed for counseling.
Counsel residents and families in dealing with feelings about death or dying and other emotional, mental, environmental or physical limitations.
Assess and complete the sections of the (MDS) Minimum Data Set assigned completely, timely and accurately.
Document the social care component of the Comprehensive Care Plan for each resident as identified by the interdisciplinary team within 7 days of completion of an MDS.
Document psycho-social needs, interaction and follow-up action(s) with resident and/or family according to facility policies and procedures.
Assess, plan and document residents' discharge needs in accordance with facility discharge planning policy and regulatory requirements.
Act as a liaison with social, health and community agencies.
Refer residents to social, health and community agencies and complete accurate documentation in residents' records concerning the results of such referrals.
Coordinate family and community resources as needed to solve financial needs and to promote emotional security.
Take part in facility in-service programs as required.
Organize community events related to resident's needs and interests.
Explain residents' rights to residents and residents' representatives.
Participate in admission of residents as assigned.
Plan, schedule and implement room visits for residents unable to leave their rooms.
Organize and participate in the theft loss program.
Coordinate and cooperate with the activity department to plan and conduct events to meet socialization needs of the residents.
Assist residents with decisions regarding advanced directives.
Coordinate room changes for residents.
Inform residents and families of Medicare benefits and send out cut letters as necessary.
Observe all facility safety policies and procedures.
Accept assigned duties in a cooperative manner.
Perform other related duties as directed by his/her supervisor.
Perform all duties assigned in an effective, timely and professional manner.
Observe infection control procedures.
Follow Residents' Rights policies at all times.
Be responsible for the safety of residents under his/her supervision.
Observe all facility policies and procedures.
Assume accountability for all data contained in the employees' handbook.
Must be able to speak, write and understand English. Excellent verbal and written communication skills are necessary.
Must possess a positive attitude toward the elderly.
Must be able to lift 20 pounds occasionally. Finger, hand dexterity, visual and aural acuity is necessary. Position requires sitting, standing, stooping, walking, bending, pushing, pulling, stretching, lifting, grasping, and reaching. Must perform job related travel as required.
Must be a licensed social worker.
One year of supervised social work experience in a health care setting working directly with individuals.
Must be at least 21 years of age.
A Bachelor's degree in social work or a Bachelor's degree in a human services field, including but not limited to sociology, special education, rehabilitation counseling and psychology from an accredited college or university.
