Job Summary: The position is responsible to plan, organize, develop and coordinate the social work and admissions operation of the nursing home in keeping with Federal and State Guidelines.

Duties include but not limited to: Coordinates all referrals in the pre-admission process for screening by the Long Term Care/Short Term Rehabilitation Charge Nurses. Responsible for securing the necessary financial information and the discharge plan for each new admission. Completes all social work and admissions paperwork and the 72 hour survey report with patient/resident/family. Provides the initial assessments and care planning needs within 24-72 hours of admission and the ongoing assessment and monitoring of each patient/resident’s psychosocial well-being. Coordinates the discharge process for all STR patients’ (or if applicable, LTC residents’) needs including the interdisciplinary patient/family discharge meetings. Provides emotional support, bereavement and crisis interventions, problem-solving mediation and facilitates resolution for patients/residents & works collaboratively with Interdisciplinary Team.

Qualifications: Graduate of an accredited university with a Masters of Social Work. At least two years experience in Long Term Care. Current NYS Social Work License recommended.
