The Social Worker, in collaboration with the interdisciplinary team (IDT), provides and facilitates acquisition of economic, social, and emotional support services to hospice patients and their families/caregivers. The Social Worker’s responsibilities include, but are not limited to: psychosocial consultation to other agency personnel; collaboration with other IDT members; referrals to outside agencies; homemaker services; and direct services to hospice patients and their families/caregivers.
This coverage area for this position will be between Mt. Pleasant and Big Rapids.
A Master’s Degree in Social Work (MSW) obtained from a school of social work accredited by the Council of Social Work Education required.
Possession of a current license to practice social work issued by the State of Michigan. A Licensed Masters Social Worker (LMSW) is highly preferred, but will consider a Limited Licensed Masters Social Worker (LLMSW).
A minimum of one (1) year paid social work experience required; hospice or related health care experience preferred. Pediatric social workers must have at least two (2) years paid social worker experience working with children and caregivers, and a basic knowledge of child development and associated behaviors related to illness.
Must be able to cope with family/caregiver emotional stress, cultural diversity and be non-judgmental of individual life-styles.
Demonstrated ability to maintain accountability and work independently while functioning as part of a cooperative and coordinated team.
Must be able to read, write and speak English fluently and be able to effectively communicate orally and in writing in internal and external relationships for all essential job functions.
Ability to use technology (hardware and software) HOM requires clinicians to use in their practice such as computer, cell phone, printer, pager, fax machine, copiers, etc. and electronic documentation software (data entry for documentation, uploading and downloading).
