Overview:

Responsible for the coordination of social services for patients/clients and their families. Collaborates with medical staff, hospital departments and community agencies to ensure efficient and effective utilization of medical social work and community resources. Coordinates bereavement services for survivors. Provides services in accordance with the mission, vision and values of Hancock Regional Hospital.

Responsibilities:

Remains calm and supportive in crises and other stressful situations. Communicates effectively with community agencies, team members, patients and families. Courteous, friendly and willing to assist others. Demonstrates organizational skills.

Associate is capable of delivering care/service appropriate to patients/customers. Plans, implements and supervises financial programs. Submits monthly reports of financial applications and programs to the Team Leader. Makes appropriate referrals to community resources. Develops and maintains positive working relationship with leaders in community resource groups. Represents the hospital at meetings and on various committees which address social needs of patients. Develops and maintains thorough knowledge of referral resources throughout the service area. Provides support services to Hancock Regional Hospital. Provides counseling/grief support to hospice patients/families. Provides inservice education and consultation in understanding the social and emotional factors related to health problems for members of the healthcare team. Provides educational programs for community and associate staff. Maintains social service program through ongoing development, implementation and/or revision of policies, procedures and guidelines for social service. Coordinates/facilitates the Hospice Bereavement Program. Assists/participates in coordinating transition planning across the continuum. Participates in IDT meetings. Provides education and assistance regarding advance directives to patients/families. Maintains accurate and timely records, including clinical and progress notes, according to department policy. Works closely with all members of the healthcare team in the development of the transition plan based on the psychosocial assessment and accepted social work interventions. Provides crisis intervention as appropriate. Contributes to planning, implementing and supporting progressive change. Makes recommendations for improvement. Actively participates on teams as requested. Maintains membership in professional organization. Maintains up-to-date knowledge of field by participating in continuing education activities. Maintains up-to-date knowledge of Medicare, Medicaid and other third-party payers. Interacts with patients and families in positive and non-judgmental manner, respecting their rights and preserving their dignity. Readily offers assistance to families, visitors, patients/clients and colleagues. Confronts individuals/situations assertively and non-aggressively when necessary. *CB*

Qualifications:

Masters degree in social work from accreditated school of social work. Mandatory Continuing Education: Customer Service, Fire and Safety, Corporate Compliance (including Confidentiality), Infection Control, and education required by regulatory, accreditation bodies, scope of practice, and/or Hancock Regional Hospital.
