Bachelor’s or Master’s degree in Social Work required. Licensed as a Social Worker in the State of Michigan preferred. Two years of experience in social work. Experience with older adults and/or persons with disabilities preferred.
Assisting residents to maintain their highest level of psychosocial functioning and wellbeing. Contingent (under 24 hours per week) 1st shift.
Maintain current knowledge of, and advocate for resident rights. Advocate on behalf of residents to staff, family and organization.
Participate in the care plan development and implementation of each resident. Provide intake processing as needed.
Work with referral sources to identify potential candidates for the center.
Serve as a key member of the resident care team.
Document on resident charts the plans, goals, and observations of each resident.
Coordinate discharge of residents, making referrals for equipment and services as needed.
Participate in monthly Quality of Life meetings with emphasis on behavior management.
Responsible for completion of assigned sections on MDS for each resident.
Compile necessary reports and statistics and submit to supervisor.
Participate in performance-related goal setting for growth and development.
Attend and participate in LSSM trainings and other professional development activity.
Other related duties may be assigned or added.
Ability to work within the Mission of the organization: Serving people as an expression of the love of Christ. Satisfactory verification of criminal/abuse check. Satisfactory Sex Offender’s Registry check. FBI Check, if applicable. OIG Registry clearance. Reliable ongoing personal transportation.
