Provides information referral and consultation and assists in the discharge planning process. Position requires master's prepared, licensed social worker or master's prepared licensed professional counselor. Plans, organizes and implements social work services for patients and families.
Knowledge of clinical social work practices and regulation standards within a healthcare setting. The knowledge base of psycho-social assessment and intervention, continuum of care and case management methods and standards, excellent communication and documentation skills. Demonstrates knowledge of Age-Specific Criteria, National Association of Social Work (NASW) Standards of Practice.
Masters prepared social worker. Two years experience in medical social work.
Active license in social work in any state AND
licensed social worker (LMSW) completed
within 90 days of hire.
MIDAS and Curaspan to be successfully completed within first 90 days.
Three years experience in an acute care setting. LCSW, Accredited Case Manager (ACM).
