Under close supervision, carries a select and difficult caseload in child or adult services including Child Welfare and Child Protective Services, In-home supportive Services and Adult Protective Services. Provides professional assessments, and support and case management services to these at-risk populations in Butte County.

· Carries a caseload of individual clients and/or families.

· Determines the urgency and severity of risk and makes recommendations regarding appropriate actions to be taken.

· May provide Immediate Response/Emergency Response (IR/ER) interventions, assisting individuals in crisis situations.

· Assesses the needs for social services of clients through home, community and office visits; collects and assesses client information through phone and personal contact; determines needs in areas such as mental health, physical disabilities, nutrition, socialization, health care, finances, domestic violence, substance abuse, and personal and family dysfunction.

· Determines clients' eligibility, and refers client to community agencies based on needs and problems identifies in the assessment; coordinates multiple services as required; assists client in accessing private and public resources and services; follows up on referrals made and actions to be taken by the client; maintains thorough case documentation for each client.

· Develops an individualized case plan with each client; monitors and follows-up regularly to reassess client status.

· Educates clients on social and health issues and the availability of community resources to meet their needs; provides support and advice in solving socioeconomic, health and family problems; maintains thorough case documentation for each client.

· Provides case management services for clients and families; assists victims in obtaining orders of protection and navigating through legal and court processes and investigations.

· Updates and maintains a variety of files, records and other documents; gathers, compiles and synthesizes data for reporting purposes; develops summary records and prepares reports, including court reports.

· Participates in multi-disciplinary team meetings to review client progress, concerns, and recommendations; prepares and presents progress reports to the court.

· Recognizes cultural and linguistic differences of client populations; provides service delivery to appropriate public, private or community services or resources, and provides language interpretation services as needed.

· Supports the relationship between Butte County and the constituent population by demonstrating courteous and cooperative behavior when interacting with clients, visitors, and other employees.

· Adheres to the standards of the Privacy Act of 1974, {U.S.C. § 552A} as amended; maintains and enforces all aspects of confidentiality of client information.

· Performs other related duties as assigned.

Minimum Qualifications:

· Master’s degree in Social Work.


· Bachelor's degree in Social Work or Behavioral Sciences; AND

· One (1) year of work experience in a public or private social services agency working with children and/or adults, or one (1) year experience performing duties comparable to an Employment Case Manager in a California county welfare department.

Upcoming graduates may supply a letter from their attending university stating impending graduation date of their degree program and verification of credits needed for graduation, which will allow placement on list of eligibles. Actual hiring of position will not occur until after graduation date.

Internship from an accredited college or university within the Behavioral or Social Services field, upon completion of: 2 semesters, may be substituted for 6 months of work experience required; OR

2 semesters in a public child or adult protective services agency may be substituted for 1 year of work experience required.

· A valid State of California driver's license is required.

Environmental Factors and Conditions/Physical Requirements:

· Working conditions are various home and office settings; involves the potential exposure to hostile situations; some non-standard work hours are required.

· May be exposed to hazardous chemicals, drugs, and infectious and communicable diseases.

· May be required to lift and carry items weighing up to 40 pounds.
