POSITION PURPOSETo provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of discharge planning. This position is accountable to the Director of Quality/Risk Management.SUMMARY OF MAJOR AND ESSENTIAL FUNCTIONSProvides hospital discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment. Coordinate the integration of the social service function into patient care. Coordinate the hospital activities concerned with discharge planning. Adhere to departmental goals, objectives, standards of performance, and policies and procedures. Ensure compliance with quality patient care and regulatory compliance.POSITION QUALIFICATIONSEducation: Bachelor's Degree in Social Work. Preferred Master Degree in Social Work in the state of Texas.Experience and Qualifications: Minimum of two (2) years of social work experience. Minimum of one (1) year experience in discharge planning from a hospital is preferred. Maintain current professional licensure in nursing or professional field of certification.Licenses/Certificates: Must hold clinical licensure as BSW or MSW or LISW in the state of Texas.BLS certification preferred. GENERAL RESPONSIBILITIES1. Provides hospital discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment.2. Coordinate the integration of discharge planning into patient care, discharge, and home planning processes with other hospital departments, external service organizations, agencies and healthcare facilities.3. Adhere to departmental goals, objectives, standards of performance, and policies and procedures.4. Ensure compliance with quality patient care and regulatory compliance.5. Evaluates the patient's medical or physical condition to assess the patient's needs.6. Investigate abuse or neglect cases and take authorized protective action when necessary.7. Promote effective and efficient utilization of clinical resources.8. Communicates with physicians at regular intervals throughout hospitalization and develops an effective working relationship. Assist physicians to maintain appropriate cost, case, and desired patient outcomes.9. Must be sensitive to patient needs including those of an ethical and cultural nature.10. Identifies actual and potential delays in service requests or treatment and communicates about these to health care team. Works with other team members to plan appropriate/timely discharges.11. Conducts interviews with patients as family members as applicable in order to get relevant psych-social information.12. Make referrals to relevant community agencies.
