Responsible for providing discharge planning & psychosocial interventions to patients & their familes.
Qualifications:
Masters Degree from an accredited school of Social Work or BSW plus 3 years experience in medical settings & enrolled in MSW program preferred. State of IL Licensed Social Worker preferred. Must have good knowledge of theory & practices of social work. Must demonstrate an understanding of community health, welfare, and social agencies.
