Provides assessment, services coordination, and counseling to geriatric clients as well as providing general support in aiding the clients in managing life situations; e.g. budget, medical care, shelter, recreational and educational activities, social services and health programs. Provides case management services to clients.

Knowledge, Skills And Requirements

1.Provides: casework/case management services to clients, including: assessment of clients’ social, emotional, and environmental status; information and referral; applications for entitlements and benefits; advocacy on behalf of clients; supportive counseling services.

2.Develops case plans and arranges for, and monitors, provision of services.

3.Organizes, facilitates, and assists at social, recreational and other group activities and events for clients.

4.Performs client intake duties by phone or walk-in. Takes information from client or collateral to determine eligibility and need.

5.Becomes knowledgeable about entitlement programs and benefits for our specific client population.

6.Maintains case notes, records, and required documentation according to departmental standards and/or regulatory requirements.

7.Prepares statistical and other reports, as required.

8.Works with Tenant Council on building/community issues.

9.Participates in regular supervision and staff meetings.

10.Acts as a team with other housing staff for events, projects, coverage and enhanced activities.

11.Supervises and coordinates activities of students and volunteers.

12.Performs other related duties as required or assigned.

Education: Master's degree in Social Work (M.S.W) or related field. Experience in geriatrics preferred. Supervision in Field Instruction (SIFI) preferred.
Bi-lingual Chinese or Korean desired.
