Plans, develops, mentors and implements in-patient diabetes management and self-management education (DSME) for patients, families and nursing staff. Develops, revises and implements diabetes education curricula and CE offerings according to identified learning needs. Facilitates competency programs that are consistent with a team approach to diabetes management and education. Collaborates with community resources, medical and nursing staff on patients referred for DSME. Participates in the Diabetes Steering Committee, policy and procedures, patient education, nurse councils, quality and performance improvement and other related activities as may be determined and may apply to diabetes management and education. Provides feedback and solutions to opportunities for enhancing staff performance, clinical competence and professional development. Facilitates an environment that values and enhances learning, knowledge acquisition and professional development.
Position Minimum Requirements
Master’s degree in nursing or health related field required. Current Florida RN license, BCLS, and CDE within 2 years of hire required. Less than one year Diabetes Management and Self-Management Education. 1-2 years teaching experience and 3-5 years clinical experience.
