Ability to align all Case Management activities with the Norwalk Hospital Strategic Plan and goals, and coordinates patient care and flow that is necessary, safe, efficient and appropriate. Excellent communication skills (oral and written). Ability to multitask and prioritize. Demonstrates leadership skills and a desire for professional growth. Applies critical thinking skills and works autonomously. Strong team player. Addresses limitations in skills and knowledge by initiating self development plan of action. Demonstrates self motivation, maturity in actions and communications and the ability to accept constructive criticism.

Case Management Activities: Transition Planning, must have the ability to assess patients' needs for next level of care and develop an appropriate plan for discharge and/or transfer. This plan must be reassessed throughout the patient;s stay, utilizing a patient centered approach in collaboration with the health care team, to ensure a comprehensive and realistic plan. Strong knowledge base of community resources. Establish successful collateral contacts. Clearly documents ongoing status of plan to ensure ongoing communication to the health care team, patient and family, while meeting all regulatory requirements. Participates in the management of length of stay through facilitation of care. Daily variance monitoring (reports/tracks and trends) while ensuring timely interventions regarding those variances. Dedication to achieving lowest length of stay within safest parameters.

Utilization Management: Makes admission and level of care determinations for Medicare/Medicaid and commercial payors regarding acute inpatient, observation, etc. Issues appropriate notices as prescribed by regulatory agencies. Performs concurrent reviews to determine appropriate status. Provides timely reviews to insurance carriers. Assists in proactive denials management.

Job Requirements:

MSW required, LCSW preferred, or willingness to obtain license as soon as eligibility requirements are met. Clinical social work experience in an acute care hospital preferred, or other experience in a medical related environment. Case management certification preferred.

Additional Job Requirements:

Must be able to deal with a high degree of public and patient contact and interaction. Must be able to make independent decisions, perform concurrent tasks, manage stressful situations and balance multiple priorities.
