Responsible for assisting the Director Clinical Services in the development, implementation and evaluation of care coordination processes and programs aimed at achieving the triple aim.
Accountable for overseeing the day to day operations of the practice care coordination staff in
supporting of the practices in meeting care effectiveness and efficiency goals through adoption of population health management The supervisor will also be responsible for a small caseload of
Practice Care Coordinator responsibilities and duties.
Bachelor’s degree in healthcare or human services required. Licensed registered nurse in the State of New York required.
Minimum 2 years clinical supervisory experience with demonstrated ability to lead a team,
build ownership, foster a positive, effective and efficient work environment.
Minimum three (3) years clinical experience in case, utilization, or disease management required.
Solid PC skills required (MS Word, Excel, PowerPoint); electronic medical records experience required
Minimum of three (3) years experience at a health plan preferred.
Demonstrated strong leadership, analytical, coaching and mentoring skills.
Demonstrated ability to clearly define and articulate goals.
Ability to motivate others and embrace change in pursuit of continuous improvement.
Demonstrated ability to identify barriers and issues and apply creative problem solving skills
and decisiveness to overcome resistance, or to make solid recommendations to senior management.

Ability to analyze and interpret data and information to identify the health needs of the population served. Demonstrated ability to use data to drive decisions and focus interventions.
Solid verbal, written and interpersonal communication skills with the ability to communicate professionally and effectively.
Demonstrated ability to manage multiple priorities and manage time effectively in a fast paced environment.
Ability to design, implement and evaluate interventions designed to improve health status.
Flexibility and resilience to support continuous change for process improvement.
Experience with clinical platforms preferred.
Ability to assume responsibility and maintain confidentiality.
Local travel among practices required.
Ability to work flexible schedule.
Maintains confidentiality and uses only the minimum amount of protected health information (PHI) necessary to accomplish job related responsibilities.
