The Support Facilitator is responsible for providing individual-specific evaluation/planning and consultation to help support the goals of the Intellectual/Developmental Disability (I/DD) service system at Cardinal Innovations serving the following counties: Alamance/Caswell; Person/Orange/Chatham; and Granville/ Vance/Franklin/Warren, Halifax; and Mecklenburg.
Incumbent is responsible for providing supports-oriented assessment/evaluation to an assigned group of individuals supported by Intellectual/Developmental Disability (I/DD) Care Coordination Department (CCD) and for serving as a consultant to the individual and his/her planning team. The Intellectual/Developmental Disability (I/DD) Support Facilitator is responsible for analysis of data related to assess support needs and services/supports being provided and, where indicated, for providing targeted consultation/co-facilitation of person centered planning meetings. The DD Support Facilitator provides leadership and consultation to supervisors, I/DD Care Coordinators, individuals/families supported, and other team members to promote movement of individuals to better and less restrictive services and supports which support individual’s needs and their desired life goals. This position requires a dynamic, proactive approach to quality enhancement in the service delivery system. The Support Facilitator spends a considerable amount of time in the field and completes required documentation/paperwork at an office location or in the employee’s home (if employee has the ability to do so at own expense).
Desired Skills & Abilities
A Bachelor’s degree in Special Education, Psychology, Social Work or closely related Human Services area and 4 years professional experience in field of developmental disabilities, preferably with consultation and program planning experience.
Must have experience with supports-oriented assessment, consultation/facilitation skills, data analysis, and support planning.
Incumbent must have demonstrated ability to successfully meet minimum requirements of the Support Facilitator position.
This is a mobile, community based position
which primarily occurs during normal business hours. Cardinal Innovations reserve the right to call upon this incumbent during non-business hours as needed to meet the needs
of the consumers and their families.
