Assists with the development, management and implementation of a comprehensive Care Plan for members currently enrolled in the program. Tracks patient compliance, identifies barriers to care, educates and assists with access to needed services.

High School diploma or equivalent required. Medical Assistant or LVN preferred. Minimum 4 to 5 years previous experience in physician’s office or hospital setting which included direct care or interaction with patients and families. 2 years previous experience with geriatric or hospice service providers preferred. Knowledge of medical terminology, strong computer skills, excellent telephone and people skills. Must be detail oriented and able to continually re-prioritize multiple tasks. Bilingual preferred, fluent in Spanish and English (written and verbal communication). No driving required.
