In response to our growth, we are currently looking for our next experienced Case Management Specialist at our new Free Standing Emergency Department !
Purpose of Position:
Under the direction of the Director, Clinical Resource Management, this clinician performs and coordinates case management functions including utilization management, discharge planning, analyzing medical records, and abstracting clinical information., He/She is responsible for psychosocial assessment, identifying, interpreting and mobilizing resources necessary for discharge planning of patients admitted to Kadlec Regional Medical Center. Provides crisis intervention and referral to appropriate community counseling agencies for patients and families that require additional clinical support. Educates physicians and staff about case management process, focusing on utilization review/management issues.
-Master’s degree in Social Work or equivalent from an accredited program.
-Requires two or more year’s recent clinical social work and discharge planning experience in an acute care hospital.
-Prefer case management and health care utilization review.
-Registration as a Social Worker (MSW) in Washington State. If Master’s in “equivalent” education, must be registered as a counselor (RC, CG, LH) in Washington State.
-Current Basic Life Support (BLS) card.
