For more than 23 years, Assisted Home Health & Hospice has delivered quality care to patients and their families. Our Santa Barbara office currently has an immediate need for a full time LVN Coordinator position.

Job Summary:
This position serves as part of the Home Health team under the direction and supervision of the Registered Nurse Clinical Supervisor or Director of Patient Care Services. The coordinator is responsible for all matters pertaining to the daily coordination of patient/family services provided by the Home Health Agency team; Intake staff cases; Insurance verifications;

Job requirements:
Current California LVN License
(2) Two years of patient coordination experience or equivalent in home health, hospital or working in a medical group.
Time management and scheduling ability are key. Ability to multi-task projects; Intake patient information; and staff a case as required.
Must have great customer service skills and critical thinking ability.
