Primary Care ACO seeks motivated, passionate entrepreneurial RN Case Manager (NP preferred) to improve quality, coordinate care, and empower our ACO patients through a patient-centered education-based model. Position will work with CMO to assess patient population, develop and monitor treatment plans, educate patients, develop and manage interventions, and coordinate care with community providers and stakeholders. The right candidate must have the ability to quickly build trust and rapport with both the assigned Medicare patients and their primary care physicians. This position requires innovation and creativity to do things better. We are looking for someone with creative ideas to improve the healthcare system in New York for seniors. Primary PartnerCare® is a company built on the patient-doctor partnership. This position is instrumental to further supporting and strengthening that patient-doctor relationship. Candidate must believe in the strength of private practice.

Essential Functions:

  • Developing & implementing case management care programs with measureable goals and outcomes;
  • Drafting policy, procedure and innovative technology tools for patients and physicians;
  • Overseeing service and communications with primary care physician, patient, family, specialists, and all other providers;
  • Assessing patients’ physical and social needs and barriers, and as the patient's advocate identifying potential solutions/resources;
  • Encouraging patients to follow prescribed course of care by serving as clinical resource and health coach;
  • Utilizing reports and working with interdisciplinary team to create reports to best monitor patient care, effective resource utilization, care coordination needs and opportunities for quality improvement;
  • Ability to see patients in their homes or any home facility (assisted living, SNF) as necessary;
  • Continuous improvement and innovation to develop new and better ways to provide care, service, education to our Medicare patients


  • Valid NYS NP/RN license and Bachelors Degree Nursing or related field;
  • Minimum 10 years clinical nursing experience in an area such as med-surg, critical care, home health;
  • Three years experience in case management, utilization management or quality improvement in a managed care setting;
  • Valid NYS drivers license, vehicle, auto insurance and clean driving record;
  • Knowledge of ICD-9s, CPTs; evidence based clinical guidelines, and quality metrics
