Interviews patients and their family members to obtain an age-specific psychosocial assessment. Assists patients and families in adjustment to illness and disabilities, and resolving personal financial and environmental difficulties, which interfere with the care management process. Provides crisis intervention, individual and family therapy where skilled social work judgment is required. Directs access to appropriate community and adjunct resources that foster improved quality of life.
Minimum Job Qualifications:
Masters Degree in Social Work from an accredited school of social work
: Must have a good knowledge of the principles and practices of social work. Will be expected to utilize a variety of highly developed skills in individual and/or group methods. Must have a good knowledge of the impact of illness on the patient and family as well as an understanding of the age specific needs and cultural differences. Work requires an in-depth knowledge of the interdependence of care management processes necessary to maximize the patient psychosocial outcome. Must possess high level of analytical abilities necessary to identify and plan for the resolution of difficult psychosocial illness related problems. Must possess high level of interpersonal skills necessary to provide counseling to patients and their families on psychosocial illness related problems.
Master’s Social Worker License in the State of Michigan. If working under a limited license, full licensure must be obtained within 6 years as required by the state of Michigan.
Skills & Abilities:
Excellent verbal and written communications skills; work processing skills; analytical decision making and judgment. Knowledgeable of social resources available to patients, both inpatient and outpatient. Strong analytical and problem solving skills. Ability to manage multiple tasks. Moderate computer skills.
