ASSOCIATES DEGREE OR BACHELORS
  • POSITION SUMMARY : Responsible and accountable for coordinating the care of patients meeting case management criteria across a continuum of care. Ensures and facilitates the achievement of quality, clinical and cost outcomes. Negotiates, procures and coordinates services and resources needed by the patient and family. Incumbent must demonstrate an ability to apply age-appropriate principles of communication and clinical practice relevant to neonatal, infant, pediatric, adolescent, adult and geriatric patients. Incumbent must demonstrate competence in the use of equipment and application of procedures before being allowed to perform independently. No call required.

    EDUCATION, CREDENTIALS, TRAINING and EXPERIENCE :

    Minimum Required: Graduate of an accredited school of professional nursing, with current license to practice as a Registered Nurse in North Carolina . Thorough knowledge of nursing care process from admission through discharge, gained through several years of clinical experience as a RN.

    Preferred: Baccalaureate Degree in Nursing and previous experience as a Care Management Specialist. Knowledge of utilization review and discharge planning methods and procedures. Understanding of clinical pathways and disease management models. Bilingual a plus. If you currently don't hold a BSN degree, you will be required to gain one in 5 years of your employment with Southeastern.
