We are currently seeking two Nurse Practitioners:
The qualified Nurse Practitioner shall be trained as a diabetic specialist. The selected candidate will work in the Adults Department managing a panel of diabetic patients with their time divided between patient visits and case management.
We are also seeking a Nurse Practitioner to work in the Pediatrics Department, a supportive group of physicians and nurse practitioners.
The successful candidates will be responsible for working collaboratively with physicians and other health care professionals to provide primary health care to acute and chronic stable patients. Obtains histories, performs physical examinations, diagnoses, treats and monitors chronic diseases, orders and interprets diagnostic studies, prescribes and selects medications, immunizations and other treatments.
The ideal candidate will possess excellent interpersonal skills to interact effectively with professionals, administrative staff, clients and representatives of community agencies. EPFMC offers a flexible schedule, competitive salary, and a comprehensive benefits package.
Duties include (but are not limited to):
Educates and counsels patients to promote positive health behaviors and disease prevention.
Performs Case Management for Diabetic Patient Populations (Nurse Practitioner, Adults)
Performs and completes routine physical and follow-up exams.
Diagnoses and treats acute and chronic medical problems.
Orders diagnostic tests, x-rays, therapeutic injections and immunizations as determined by the symptoms or illness of the patient.
Work closely with clinical staff in coordinating and obtaining needed services for patients and families.
Keeps currently dictated or written medical records on and records all pertinent information regarding patient history, physical diagnoses and treatments accurately.
Refers patients to specialists in accordance with EPFMCs referral policy when deemed necessary.
Uses, protects, and discloses EPFMC patients protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
Consults with attending physician as necessary.
Performs additional duties as assigned.
Master's Degree in Nursing
Proficient in English and Spanish
Current CPR Certification
Possess current certificate of completion from a Nurse Practitioner program, current NP License required
Experience in Internal Medicine or Family Practice, preferred for Adults Department
Experience in Pediatrics or Family Practice, preferred for Pediatrics Department
Must be able to work in interdisciplinary teams
Must have at least one to three years of experience as a Nurse Practitioner
