General Summary

The role of the Electronic Medical Records (EMR) Abstraction Specialist is responsible for reviewing paper medical record for pertinent information (i.e. patient allergies, medications, past medical history, family/social history and procedures) that will be abstracted for the electronic medical record. Enters information found into the electronic medical record (EMR) software. Reports to the Director of Health Information Management and/or the Medical Records Manager.

Uses and discloses patient protected health information: 1) Only as it applies to job functions, 2) in amounts minimally necessary for intended purpose, and 3) in a confidential manner.

Principle Duties and Responsibilities

Essential Job Functions

Reviews and analyzes paper medical records to identify pre-defined, pertinent patient information for abstraction into the EMR.

Performs accurate and complete abstraction of patient medical records following EMR abstraction guidelines.

Consults with provider staff and/or EMR Abstraction Lead if questions arise regarding information to be abstracted.

Performs accurate scanning of identified documents into appropriate section of the EMR as needed

Participates as a team member and providesleadership to colleagues, students and other team members to ensure that workloads are distributed fair and equal and that our number one goal of providing excellenceof service and care to our patients and their families is accomplished every day.

High school graduate or equivalent.

Certified Medical Assistant, Licensed Practical Nurse or Apprentice Coder

Experience or Pre-Requisites:
1.Knowledge of medical terminology.

2.Knowledge of proper grammar, spelling and punctuation.

3.Knowledge of Microsoft Office software, such as Word, Excel and Outlook.

4.Ability to spell and alphabetize.

5.Good listening and communication skills.

6.Good telephone and customer service skills.

7.Previous medical office experience preferred.

8.Strong computer background.

9.Familiarity with health insurance plans, including Medicare, Medicaid, and managed care helpful.

10.Good vision and hearing.

11.Strong attention to detail.
