The PHN (Public Health Nurse) position involves public health nursing work to promote the health of individuals, families and populations using knowledge from nursing, social and public health sciences.
Work involves visiting clients and families in the home, office, or clinic to assess health and psycho-social needs, provide health education, and refer to community resources. Work also involves conferring with various community based agencies to promote department and interagency program coordination.
BSN and minimum one year related work experience. The ideal candidate will have experience working with children and youth with special health care needs.
Must be licensed as a Registered Nurse (RN) in Colorado.
Must have valid driver's license, if you are from out of state, you must obtain a valid Colorado driver's license within 30 days of hire date and you must not have any major violations, nor DUI, DWI, DWAI conviction in the past three years.
Must provide immunization record at time of hire or prove immunity to comply with departmental Communicable Disease Control Policy.
