Director of Case Management and Social Services Providence Health & Services -
San Pedro, CA
Direct all aspects and activities responsible for monitoring the delivery of patient care and appropriate discharge planning.
Oversee the collection, analysis, and reporting of financial and quality data related to utilization management, quality improvement, and performance improvement.
Plan, organize, analyze, develop and implement Departmental programs, services and strategies to ensure favorable Hospital operating results.
Facilitate the growth and development of the case management program and social work program consistent with Providence’s philosophy and in response to the dynamic nature of the health care environment through benchmarking for best practices, networking, quality management and other activities as needed.
Required qualifications for this position include:
Bachelor’s Degree in Nursing (BSN).
Current Registered Nurse (RN) licensure in the state of California.
Ten years multidisciplinary acute care/clinical experience.
4 years case management experience.
2 to 5 years management leveled experience.
Experience in utilization management, case management, discharge planning, disease management and other cost/quality management program.
Current BLS certification for Healthcare Providers from the American Heart Association
Preferred qualifications for this position include:
Master’s Degree in Nursing (MSN)
Case Manager (CCM) or Accredited Case Manager (ACM)
