Responsible for the coordination, integration and facilitation of care and services for the patient by all the members of the health care team. Provides appropriate social services to patients, families, and others identified as having social, psychological, and/or environmental needs related to their behavioral health admission or disease process. The social work care coordinator is responsible for the evaluation of patients; development, coordination, and implementation of the treatment plan; maintenance of informative, accurate, and timely documentation; participating in team meetings and coordinates services for discharge planning and referrals to appropriate community agencies.

Qualifications/Experience

Graduate of a Social Work Program, MSW required. 3 to 5 years experience. Current Iowa LMSW. Knowledge and skills necessary to provide services/care appropriate to the age and diagnosis of patients served across the continuum of care, in accordance with the hospital/departmental/unit standards. Maintains working knowledge of applicable Federal, State, and local laws and regulation as well as other policies and procedures in order to ensure adherence in a manner that reflects honest and professional behavior.
