This Nurse Practitioner (NP) position requires making home visits, performing annual health assessments, reviewing medical histories, medication reviews, depression screenings, educating on and encouraging healthier lifestyle choices.
Required:
Nurse Practitioner (NP) with an active, unrestricted license to practice in Indiana
2+ years Nurse Practitioner (NP) working experience
Preferred:
Experience in gerontology, cardiology, internal medicine, or endocrinology a plus.
Home Health or Home Visit experience
