Per Diem Hospice Registered Nurse - Home Healthcare RN

Job Responsibilities
As a Per Diem Hospice Registered Nurse for Caring Hospice, you will perform ongoing patient assessment, maintaining thorough and diligent notes and records regarding the effectiveness of their care plan. You will also keep in constant contact with patient family members and alternative caretakers.
Additional responsibilities for the Hospice Registered Nurse Case Manager include:
- Providing nursing care with an emphasis on symptom management
- Supervising professional home health aides
- Referring patients’ families to applicable community resources in collaberation with the team
- Maintaining strict confidentiality
- Providing care directions for family members
- Requires on-call runner coverage
Requirements
Per Diem Hospice Registered Nurse - Home Healthcare RN
As a Per Diem Hospice Registered Nurse, you must have absolute integrity and sound judgment. You will act as a patient advocate, working independently to ensure your patients are physically comfortable and content. We are seeking RNs with good social skills; energetic, organized and emotionally capable of caring for terminally ill individuals. You must also be a team player, able to work harmoniously with a wide variety of people.
Additional requirements for the Hospice Registered Nurse include:
- Being licensed as a Registered Nurse in the state of Connecticut
- 1-2 years of Nursing experience
- Ability to occasionally lift 25 pounds
- Previous Hospice experience, a plus
