The Health Educator is responsible for a range of direct service and case management functions related to the delivery of health and wellness services.
SPECIFIC DUTIES & RESPONSIBILITIES
Conducts HIV and infectious disease assessment and HIV pre-test counseling session with all newly admitted residents within one week of admission.
Conducts HIV post-test counseling sessions with all tested residents upon receipt of lab results.
Conduct sexually transmitted diseases counseling session with all residents within two months of admission.
Conducts quarterly wellness counseling sessions with HIV/HCV + clients which focus on adherence.
Delivers regularly scheduled psycho-educational lectures in designated facilities on infectious diseases, tobacco dependence, medication adherence, self-care for chronic diseases and a range of other topics related to health and wellness.
Regularly attends multidisciplinary case conferences.
Assures continuity of medical care upon discharge for HIV/HCV + clients.
Assures that healthcare remains well integrated with psycho-social treatment.
Provides updates to medical providers on client behaviors which impact healthcare such as medication adherence, substance use and other behaviors which threaten continued engagement in services.
Assures adequate client access to nicotine replacement therapies in designated facilities.
Attends all required in-service training seminars.
Participate in quality improvement activities.
Other relevant duties as required.
REQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES
NYS DOH Certification for HIV counseling. LPN required
Bachelor's Level Clinical Degree and one (1) years of experience working in a social services setting or High School Diploma/GED and three (3) years experience working in a social services setting.
Proficiency with computer operation (Microsoft Word, Excel, AWARDS, and Outlook programs).
Must be able to work a flexible schedule.
