Job Summary Coordinates the care of patients admitted for hospice care in accordance with professional licensure, established procedures and regulatory requirements. Coordination of care includes: coordination and direction of the interdisciplinary team, patient, family and facility staff; patient symptom management; and assessment of patient and family physical, psychosocial, cultural and spiritual needs. The incumbent provides end-of-life education to patient, family and facility staff and advocates for patients and families to ensure patient-focused care. An incumbent exercises independent judgment consistent with professional licensure and applies skills obtained through academic and clinical preparation.

Unusual problems or proposed deviations from guidelines, practices or precedents are discussed with a manager or physician before being initiated. Essential Duties Delivers and coordinates patient care in accordance with established protocols. Assesses and monitors patient/family physical psychosocial and spiritual needs so that the plan of care reflects holistic needs. Explains hospice services and eligibility criteria, and elicits clinical information from patient and/or family regarding health status and readiness of hospice care.

Completes and communicates accurate and timely documentation of pertinent patient information. Captures patient status in accordance with Medicare standards. Communicates patient’s condition and needs to the interdisciplinary group for further care plan management. Informs referral source of patient’s admission status in a timely manner; obtains and documents information regarding advanced directives, and obtains signed DNR as appropriate; ensures appropriate documentation is completed in accordance with established standards.

Implements and revises patient Plan of Care, taking into consideration physical, psychosocial, spiritual and financial resources. Receives telephone calls via 24-hour answering service during and after normal business hours to facilitate referral activity, complete the initial certification of terminal illness form and coordinate admissions to hospice. Utilizes agency treatment protocols and algorithms to initiate urgent interventions, and communications the interactions with clients to the IDG through verbal and written reports. Works collaboratively with assigned core care team to update the Hospice Plan of Care to reflect changes to meet the needs/wishes of the patient and/or family.

Provides education to patient/caregiver and participates in staff/community education activities. Assesses learning readiness and educates patients and/or families concerning their physical, psychosocial or spiritual needs and monitors the status of hospitalized patients. Provides training to Hospice staff to enable them to excel in patient care efforts. Cultural Sensitivity and Competence: Demonstrates proper use of communication tools/materials for effective communication and understands how the culture(s) of patient populations can affect communication, collaboration and the provision of care, treatment and services.

Patient Population Served: Demonstrates knowledge and proper skills associated with the department’s defined specific populations served. Performs related duties as required.
