The Clinical Care Facilitators (CCF)/Clinical Nurse Leader (CNL) will exhibit a high level of clinical practice, leadership, critical thinking and other skills useful to improve/coordinate patient care for an adult population. Functions as a generalist at the point of care and manages and coordinates care of individuals and clinical groups. Responsible and accountable for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. The CCF/CNL will work collaboratively with the providers, nurse manager, assistant nurse manager and staff as the subject matter expert in the clinical setting. Applies the nursing process and care management principles within a collaborative, interdisciplinary practice setting, which will then enable effective attainment of appropriate lengths of stay and the effective level of care. Core functions include daily assessment of patient care needs to determine the most appropriate use of expertise in clinical coordination, patient and staff education, attention to systems issues that require intervention, the application of evidence based practice, improving nurse sensitive indicators, and quality and performance measures. Assists the nursing staff in the provision of complex patient care on the medical-surgical unit. Care is provided for patients from the following surgical subspecialties: vascular, general, orthopedics, ophthalmology, urology, plastic surgery, otolaryngology, gynecology, neurosurgery, thoracic and podiatry. Care is provided to patient with the following medical diagnoses: congestive heart failure, coronary artery disease, chronic obstructive pulmonary disease, hypertension, stroke and renal insufficiency.
Relocation expenses will not be reimbursed. QUALIFICATIONS REQUIRED:
Citizen of the United States.
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia . In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
Must be proficient in spoken and written English.
Current certification in BLS
Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. The recommended salary may be at any point in the range listed for this vacancy.

BSN with active enrollment and pre-requisites in a master's program with a clinical nurse leader major is required.
National certification from a VA approved certifying body as a Clinical Nurse Leader (CNL) is preferred.
