Unity has been a part of Northeast Wisconsin for more than 35 years. With hospice, palliative and bereavement care services extending into 12 counties, our dedicated patient care staff, administrators and advisory teams continue to guide our focus in providing the community with access to education, support and quality holistic care.

Role and Responsibilities
    • Provide grief counseling to Unity patients/families
      • Responds to counseling needs as per the plan of care. Documents all interactions with bereaved clients
      • Identifies grief-related issues and makes bereaved clients aware of available resources
    • Facilitate grief groups
      • Designs a program format consistent with the needs of the group
      • Maintains records for the program
      • Identifies needs not appropriate to be met by the group and refers to appropriate resources
    • Provide information on Unity programs and education related to grief and loss
      • Provides public speaking as requested and approved by supervisor
      • Explains bereavement program to potential clients/families as requested by supervisor
    • Provide counseling to bereaved community clients
      • Provides counseling on a limited basis as directed by supervisor
    • Make referrals to community resources based upon client needs. Participate in the planning and provision of memorializing programs
    • Comply with all applicable local, state and federal laws and regulations as well as policies and procedures of Unity
    • Satisfy mandatory training requirements regarding local, state and federal regulations and/or Unitys policies and procedures
    • Consistently and concisely document activity according to department protocols and standards
    • Promote and maintain confidentiality
    • Maintain personal and professional boundaries
    • Perform other job-related duties as assigned
    • Maintain regular and reliable attendance
    • Shares in providing coverage for evening and weekend bereavement programs and requests.
  • Bachelors degree in social work or related mental health field required; Masters degree in counseling preferred
  • Current license or certification for the State of Wisconsin
  • Previous experience in the field of grief, loss, and end-of-life preferred
    Current license or certification for the State of Wisconsin
