The Community Outreach Coordinator builds and manages customer alliances with the ability to create, nurture, cultivate, manage and protect referral relationships.

Formulates and coordinates wide-reaching educational opportunities in the community for general educational programs; joint educational programs with hospitals, ALFs, SNFs and other affiliated institutions; physicians and their staff, faith-based communities, businesses, and social organizations of major strategic significance.

Assists the Executive Director in developing annual plans for general community outreach and education to external audiences and constituencies including but not limited to: broad-based media initiatives, annual corporate events, community functions, community work projects; community boards; disease specific associations; and service functions and sponsorships.

Continues to market internally the advantages and benefits of using Evercare Hospice as part of the Evercare, Secure Horizons, Optum Health, and other UHC sister segments for the continuum of care.

Makes 10-12 quality face-to-face calls per day among the 50 identified key referral sources decided upon with the help of the Executive Director.

Completes all job duties in a manner that meets the accepted standards of practice and the Evercare Hospice & Palliative Care Policies and Procedures.

Contributes to achieving the company's mission.

Works collaboratively with others to achieve goals.

Will report to the Colorado Sales Manager and site Executive Director. Performance goals are tied to admission activity and ability to meet growth goals to support the overall success of the Denver Evercare Hospice and Palliative Care program.

Requirements Minimum of two years experience in marketing or community outreach in the field of health care.

Bachelor's Degree in marketing or related field (education, journalism, communications, etc.)or equivalent work experience.

Ability to teach, public speak and/or communicate effectively with individuals and/or groups.

High degree of skill in human relations, teaching, and communications.

Ability to oversee production of written materials used for outreach.

Excellent written and oral communication skills.

Reliable transportation with appropriate license and insurance coverage.

Excellent computer knowledge and experience.
