Responsible for the delivery of Hospice and Palliative Care Services that are therapeutically safe, effective, and efficient. Exercises independent judgment and demonstrates leadership abilities in the Supervisor role. Serves as a resource to staff, interpreting the mission and vision, policies, and procedures of the agency and health system. Leads an interdisciplinary team of healthcare professionals in providing care to seriously and terminally ill patients. Works with Home Care and Hospice Leadership Team to develop and implement strategies to position hospice and palliative care service line for continued strength and growth in the market.
Graduate of accredited Nursing Program
Current licensure to practice as a Registered Nurse in the State of Pennsylvania
Current CPR (BLS)
Valid PA Driver's License
Prior Home Health/Hospice experience and/or management experience
Strong leadership ability, independent forward thinking and decision-making ability; analytical problem solving skills, sound judgment, excellent oral and written communication skills, must be able to function in a team environment
Knowledge of Medicare Hospice Conditions of Participation and survey process
Minimum of two years experience in the past three years in a home health/hospice agency; Leadership experience in Home Care/Hospice; Microsoft Windows and related computer experience; Applicable certification by Nurses' Credentialing Center; Bachelor of Science in Nursing: Certification in Hospice/Palliative Care Nursing.
