Accountability ObjectiveThe RN/LPN provides direct care and services to clients under the supervision of the Director of Service Delivery, and in accordance with physician orders documented on the Plan of Treatment and subsequent physician orders. Nurses will follow all rules, regulations, policies and procedures of the federal and state governments, and the Franchisor standards, policies and procedures. Nurses will also conform to the standards and practice acts available to the practice of nursing in the state. In addition to providing direct care, nurses will also provide direct supervision of services delivered by paraprofessionals who may also be assigned to the client. Primary duties include: clinical treatments, coordinating effective communication with caregivers and others to assure accurate flow of information and services are delivered in conformance with the Service Plan or Plan of Treatment. Essential Duties and Responsibilities: •Builds strong relationships with caregivers and clients/families.•Educates the community, clients and families on services available.•Must document and maintain clinical records in conformance with federal, state and Franchisor rules, regulations and quality standards.•Ability to manage and coordinate a clients needs using a case management approach under the direction of a registered nurse.•Ability to implement the Plan of Treatment or Service Plan as applicable.•Accountable for paraprofessional caregiver oversight and supervision and documentation of all supervisory visits.•Provides direct client care under the orders of a physician, supervision of a registered nurse and in accordance with a Plan of Treatment.•Renders all services in compliance with quality standards established by Franchisor.•Serves on-call as needed to ensure coverage 24/7.•Works cooperatively with other CareMindersÒ Home Care franchise offices.•Must comply with Franchisor rules, standards, policies and procedures. Education: Licensed practical or vocational training in nursing; nursing license in good standing is required.Registered Nurse Education; Nursing License in good standing required.Experience:•Minimum of one (1) year as a nurse in the home care industry. •I.V. Certification for LPNs•Vent/Trach Care experience•Wound Care•Pediatrics experience•Knowledgeable in regulations of Medicare, Medicaid and Waiver programs for all services available in the homecare industry•Experience in maintaining clinical records in compliance with state, federal and quality standards and rules and regulations•Strong interpersonal and customer service skills.•Ability to work with all levels of management.•Ability to handle multiple priorities simultaneously.•Proven record of compliance with rules and regulations.
