Medicaid Service Coordinator – Full Time
Promotes patient advocacy for children with special healthcare needs through the provision of skilled professional Medicaid Service Coordination services. Ensures that a person centered planning process is used in delivering Medicaid Service Coordination, and that it is also relevant to the medical, psychological, and social needs of the child and family.
The qualified candidate will be serving Nassau and Suffolk counties but it might be a subject to change

Minimum Associates degree in health or human services field, or a RN.
Minimum one year experience working with people with a developmental disability or one year experience as a service coordinator.
Core service training preferred.
Bilingual - Spanish preferred
MUST have reliable transportation.
